The UKPM Digital Memorial remembers and memorialises fallen officers. This form can be used to provide additional information about a new or existing officer.

    • All information supplied is passed to our partner charity, The Police Roll of Honour Trust, who will check this and let us know whether to include it on the Digital Memorial.
    • Profile pictures are checked before they are made public and we reserve the right not to use any photos supplied. Only upload photographs that you have copyright, or permission, to use.
    • You retain the copyright of images and text that you upload but you give us and our partner charity, The Police Roll of Honour Trust permission to use them on our website, on social media and on any printed material.
    • You can remove this permission at any time by telling us.
